What is Gluteoplasty or buttock augmentation?
Gluteoplasty, also known as buttock augmentation, is a surgical procedure that is performed on both men and women typically between the ages of 25 and 45, who desire an increase in volume, size, or improvement in the appearance of their buttocks.
The ideal candidate for this surgery is someone with elastic skin and good buttock quality. It is important to note that flaccid skin and a lack of muscle tone may limit the results achieved. As a result, the older an individual is when deciding to have this surgery, the less noticeable the results may be. It is not recommended for patients with sagging buttocks or excess flaccidity in the gluteal region. In such cases, a different procedure may be necessary, although good results are still achievable.
The Brazilian butt lift is a popular surgical procedure where a surgeon takes fat from one part of the body, typically through liposuction, and injects it into the buttocks. This surgery has gained significant popularity in recent years and is performed by plastic surgeons.
The traditional bum lift procedure is different from the Brazilian Butt lift, as the cosmetic surgeon will make incisions across the upper buttock. Half-moon-shaped segments of skin and fat from underneath the surface of the skin are removed from the buttock area. Then, the remaining skin and tissue are lifted to achieve a tighter and smoother appearance.
A bum lift operation is designed to improve the appearance of loose or sagging skin around the buttocks. It is often combined with liposuction to remove any excess fat, resulting in a complete reshaping of the buttocks. The patient’s own fat is molded into shape and used to enhance the fullness of the buttocks.

Gluteal implants are inserted through a small vertical incision made between the two buttocks, at the level of the lower spine. This placement allows the scar to be hidden completely. The implants used for buttock augmentation are similar to those used for breast augmentation, but the silicone gel used is more cohesive and firmer to account for the unique characteristics of the gluteal area, which requires stronger implants with greater compressive strength.
It is crucial to note that gluteal implants should never be placed over the gluteal muscles (gluteus maximus). Doing so can cause them to become dislodged and potentially slide down or rotate. Therefore, the safest and only appropriate location for them is between the gluteus maximus muscles. This is typically achieved through two separate incisions in the midline of the buttock.
During the procedure, the surgeon will make an incision in the buttock fold and access the muscle. The muscle will be cut 4 cm deep to create a pocket for the implant. Once the implants are placed, the skin is sutured over the pocket to cover and secure them.

Lipofilling
Lipofilling is a surgical technique where fat is extracted from one part of the body and injected into another. The fat is usually taken from areas such as the abdomen, thighs, or hips where it has accumulated. This procedure not only provides additional volume to the buttocks but also enhances the overall body structure and proportion.
The extracted fat is purified and then injected into the buttocks. As the fat cells are autologous, i.e., they belong to the patient’s own body, there is no risk of rejection or allergic reactions. Despite efforts to improve the survival rate of the transplanted fat, a certain amount of fat is reabsorbed by the body. However, the amount of adipose tissue that does not reabsorb during the first three months typically remains stable.
To achieve the desired results, hypercorrection can be applied during the surgical intervention, which involves injecting more fat than the required volume. Nevertheless, the outcome of the procedure is usually permanent, with no further complications. Lipofilling can effectively enhance the shape of the buttocks, increase their size, and provide a natural appearance.

What to expect
During the initial consultation, all preferences and possible issues are discussed with the patient. The details of the surgery are highlighted, and the shape of the implants is selected, taking into consideration the patient’s figure type and individual features. To choose the best option, the surgeon carries out specific measurements of the buttocks. The surgeon must also inform the patient about age restrictions applicable to this procedure. Gluteoplasty is only allowed for patients over 18 years old.
The next preparation stage is getting lab testing. A comprehensive clinical examination is performed, including a mandatory blood test to detect viral diseases (such as AIDS, hepatitis, syphilis, etc.). The blood clotting ability and Rh factor are also determined.
A patient should drop bad habits (such as smoking, consuming alcohol, and overeating) to speed up the recovery period and avoid complications. A few days before surgery, a low-calorie diet is prescribed to facilitate body detoxification. Eating and drinking must be stopped fully 8 hours before the procedure, during which period the intestines are purged.
If a patient has a cold at the time of the planned procedure, it is postponed until full recovery. This plastic surgery cannot be performed during pregnancy and menstruation periods.
The surgeon will make an incision in the crease between the buttocks, where the implant will be placed. Alternatively, the incision may be made in the lower back, upper thigh, or near the hip. The incision is typically about 3-5 cm in length. The surgeon will create a pocket in the gluteal muscle to place the implant. The implant can be positioned either above or below the muscle, depending on the patient’s anatomy and the desired outcome. Submuscular placement: In this technique, the implant is placed beneath the gluteal muscle. The surgeon will carefully dissect a pocket beneath the muscle, then place the implant in the pocket. This technique can provide a more natural look and feel, but may require a longer recovery time. Subfascial placement: In this technique, the implant is placed between the muscle and the fascia (a layer of connective tissue). This technique can provide good coverage of the implant, but may not be suitable for patients with thin skin or little subcutaneous fat. Intramuscular placement: In this technique, the implant is placed within the gluteal muscle itself. This technique is less common and may be associated with a higher risk of complications. Once the implant is in place, the surgeon will close the incision with sutures or surgical glue. The incision is typically closed in layers to ensure proper healing.
Lipofilling: Only the highest quality fat cells are injected into the buttocks. The fat is harvested, purified, and prepared for grafting before being injected into the body. In recent years, this method has surpassed traditional techniques like implants to create more natural-looking results and fullness. The surgery is divided into three stages:
- The fat is removed from the body using cannula tubes.
- The fat cells are washed and prepared
- The fat is injected back into the body.
Immediately after surgery the patient is transferred to a clinic ward and kept there under doctor’s supervision for a few days. Sitting on buttocks will be impossible for the first 3 days due to pain and swelling. A patient will be able to sleep on the stomach only. In case of severe pain the analgesics are prescribed. Antibacterial therapy can be introduced to prevent infections.
Suture threads will be absorbed in 10 days. A drainage tube is removed a few days earlier. Then, a patient must wear special compression garments. It prevents swelling and reduces load on gluteal muscles.
Sometimes a mild depression can be observed due to the long postoperative period. This is normal condition which subsides along with wound healing. From time to time patients experience loss of sensation at the operated site. Sensitivity will return in 2 or 3 weeks without any treatment.
It is possible to resume a normal working schedule in 2 weeks. Along with it a patient must observe strict limitations and follow a doctor’s advice. Intense sports, tanning at the solarium, visiting swimming pools and sauna are prohibited during the month following the procedure. Smoking is forbidden for the whole recovery period. No intramuscular injections are allowed at the buttocks area.
In about 3 months the implants will be fully accepted, the tissues will heal completely, the buttocks will gain the desired shape and the scars will be gone. At that time a patient will be able to evaluate the final result of a surgeon’s work.
Frequently asked questions
This is a very important question, and you should always consult with your surgeon to determine the appropriate size for your body type and skin elasticity. Going too big too quickly can result in complications such as opening of stitches, infection, and ultimately, implant removal.
Initially, you may be able to feel the buttock implants, but they should become less perceptible over time, typically around 2 months after the gluteoplasty surgery. Eventually, you should be able to feel the implants as if they were a natural part of your body.
You should wait for at least one month after the surgery before resuming any sports activities, following the advice of your surgeon.
